Asthma is one of the most common chronic conditions in childhood.L’asma è una delle malattie croniche più frequenti dell’età pediatrica.

It’s marked by episodes in which the smaller airways, the bronchi, become inflamed and narrow, making it harder for air to pass through.È caratterizzata da episodi nei quali le vie respiratorie più piccole, i bronchi, si infiammano e si restringono, rendendo più difficile il passaggio dell’aria.

This narrowing is generally reversible, either on its own or with treatment, but it can happen again over time in response to certain triggers.

The most typical symptoms are coughing, wheezing, difficulty breathing, and a sense of tightness or pain in the chest.I sintomi più caratteristici sono tosse, fischi durante la respirazione, difficoltà a respirare e senso di costrizione o dolore al torace.

Not every child has the same symptoms, though, and especially in the first years of life, it isn’t always easy to tell right away whether we’re really looking at asthma.

How does asthma show up?Come si manifesta l’asma?

One of the most common symptoms is coughing, usually dry and recurrent.

It can show up especially at night or in the early morning, during or after physical activity, or after exposure to cold air.Può presentarsi soprattutto durante la notte o nelle prime ore del mattino, durante o dopo l’attività fisica oppure in seguito all’esposizione all’aria fredda.

Another characteristic symptom is wheezing, what’s commonly described as a “whistle.”Un altro sintomo caratteristico è il respiro sibilante, quello che comunemente viene descritto come un “fischio”.

The wheeze is mostly heard on breathing out: it appears above all when the child is pushing air out.

In mild cases it may only be picked up by the doctor while listening with a stethoscope; when the obstruction is more significant it can also be heard without any instrument.

The child may also describe heaviness, tightness, or pain in the chest.

Younger children, who aren’t yet able to describe what they’re feeling very well, may sometimes complain generically of belly pain or simply appear agitated and short of breath.I bambini più piccoli, che non riescono ancora a descrivere bene ciò che provano, possono talvolta lamentare genericamente dolore alla pancia o mostrare semplicemente agitazione e difficoltà respiratoria.

Symptoms can come and goI sintomi possono andare e venire

An important feature of asthma is how variable it is.Una caratteristica importante dell’asma è la sua variabilità.

A child can have coughing and wheezing in a particular situation and then go back to completely normal afterward.

Some children feel well between one episode and the next.

Others have more persistent symptoms, with periodic flare-ups.

In some children the episodes appear mainly during respiratory infections; in others during physical activity, or after exposure to particular allergens or irritants. In alcuni gli episodi compaiono soprattutto durante le infezioni respiratorie; in altri durante l’attività fisica oppure dopo l’esposizione a particolari allergeni o irritanti.

This variability itself is one of the features the pediatrician takes into account when considering the possibility of asthma.

What can trigger an attack?

The airways of a child with asthma can react to very different triggers.

Among the most common are viral respiratory infections, including an ordinary cold.

In some children, inhaled allergens can matter too, such as pollen, dust mites, or pet allergens.

Other possible triggers include:Altri possibili fattori scatenanti sono:

  • physical exerciseesercizio fisico
  • cold airaria fredda
  • tobacco smokefumo di tabacco
  • air pollutioninquinamento atmosferico
  • fumes and other environmental irritantsfumi e altri irritanti ambientali
  • particular weather and environmental conditionsparticolari condizioni meteorologiche e ambientali

Not every child reacts to the same triggers.Non tutti i bambini reagiscono agli stessi stimoli.

Recognizing over time the situations in which symptoms appear can therefore be very useful.

It’s useful to distinguish what can trigger an episode in a child who already has asthma from what can increase the likelihood that a child develops asthma in the first place.È utile distinguere ciò che può scatenare un episodio in un bambino asmatico da ciò che può aumentare la probabilità che un bambino sviluppi asma.

A personal or family history of asthma, allergies, or atopic dermatitis can be linked to a greater predisposition.

Exposure to tobacco smoke during pregnancy and during childhood is also an important unfavorable factor.

Pollution and other environmental factors can play a role.Inquinamento e altri fattori ambientali possono avere un ruolo.

But we can’t always identify a precise cause.

A child can develop asthma even when parents can’t point to any obvious predisposing factor.

Is a child who wheezes with a cold asthmatic?

No, not necessarily.No, non necessariamente.

This matters especially in the first years of life.

Many young children have episodes of wheezing during viral infections.Molti bambini piccoli presentano episodi di wheezing, cioè respiro sibilante, durante le infezioni virali.

A one- or two-year-old who coughs and wheezes with a cold shouldn’t automatically be considered asthmatic.

A share of these children, as they grow, will stop having episodes altogether.

Diagnosing asthma in a young child can be complex, and it’s based on the overall clinical picture: how often the episodes happen and what they look like, any symptoms between infections, the presence of allergies or a family history, triggers, and response to treatment.

When the child’s age allows it, the doctor can also turn to lung function tests.

Nighttime cough: it doesn’t necessarily mean asthmaTosse notturna: non significa necessariamente asma

A dry cough that comes on frequently at night, in the early morning, or during physical activity can raise the possibility of an asthma component.

But the cough alone doesn’t allow us to make a diagnosis.

One condition that can cause similar symptoms is, for example, rhinosinusitis.

Secretions from the upper airway can cause coughing, especially at night and on waking. Physical activity can also make it worse, by helping move those secretions along.

For a parent, it can be very hard to tell the two situations apart.

This is exactly one of those cases where a phone conversation can gather useful information, but isn’t always enough.

It may be necessary to examine the child and listen to the chest to better understand where the cough is coming from.

What should parents watch for?Che cosa deve osservare il genitore?

Parents shouldn’t try to determine on their own whether their child has asthma.

But they can give the pediatrician extremely useful information.

It’s important to observe:È importante osservare:

  • when the cough appearsquando compare la tosse
  • whether it’s mostly daytime or nighttimese è prevalentemente diurna o notturna
  • whether it appears during or after running and physical activityse compare durante o dopo la corsa e l’attività fisica
  • whether it’s brought on by cold airse viene provocata dall’aria fredda
  • whether it consistently appears with coldsse compare sistematicamente durante i raffreddori
  • how long it lasts after a respiratory infectionquanto dura dopo un’infezione respiratoria
  • whether wheezing can be heardse si sentono dei fischi
  • whether the child shows difficulty breathingse il bambino mostra difficoltà a respirare
  • whether the episodes tend to repeatse gli episodi tendono a ripetersi
  • whether there’s a personal or family history of asthma or allergiesse esiste una storia personale o familiare di asma o allergie

This information, together with the exam, can help the pediatrician a great deal in piecing the picture together.

Exercise-induced asthmaL’asma provocata dall’esercizio fisico

In some children, coughing, wheezing, or difficulty breathing appear during, or especially right after, physical exertion.

Running can be one of the triggers.Correre può essere uno dei fattori scatenanti.

This doesn’t mean a child with asthma should avoid sports.

On the contrary, a child with well-controlled asthma should be able to take part in normal physical activity. Symptoms that consistently appear during exercise are instead information worth passing on to the doctor, since it can point to a need to reassess how well the condition is controlled and the treatment plan.

When is a flare-up becoming serious?Quando una crisi sta diventando importante?

During a flare-up, we shouldn’t judge how serious it is just by how loudly the child is wheezing.

Above all, we need to look at how much work the child has to do to breathe, and at their overall condition.

⚠ Signs that a flare-up is becoming serious:Segnali che indicano una crisi importante:

  • obvious difficulty breathingdifficoltà respiratoria evidente
  • breathing much faster than usualrespiro molto più rapido del solito
  • visible use of the chest muscles to breathe, with the chest pulling inutilizzo evidente dei muscoli respiratori, con rientramenti del torace
  • a significant rise in heart rateaumento importante della frequenza cardiaca
  • difficulty speaking normally because the child has to stop to breathedifficoltà a parlare normalmente perché il bambino deve fermarsi per respirare
  • needing to sit up because breathing is easier in that positionnecessità di stare seduto perché in quella posizione riesce a respirare meglio
  • obvious agitation or distressevidente agitazione o preoccupazione
  • a feeling of heaviness, tightness, or pain in the chestsenso di peso, costrizione o dolore al torace

A very young child may not be able to explain what they’re feeling. In these cases, it’s mainly their behavior, breathing, and overall condition that should catch a parent’s attention.

This is a particularly important point.Questo è un concetto particolarmente importante.

If the airway obstruction becomes very severe, so little air may be moving that the wheeze can actually become less noticeable.

So you shouldn’t necessarily wait to hear a loud wheeze before realizing a child is struggling to breathe.

If the child seems very tired, is breathing with great difficulty, struggles to talk, or their overall condition is getting worse, the situation needs urgent evaluation.

Another situation that deserves particular attention involves a child with an already-known asthma diagnosis.

The family may already have received clear instructions from their doctor on what to do during a flare-up.La famiglia può avere già ricevuto dal proprio medico indicazioni precise su che cosa fare in caso di riacutizzazione.

But if the treatment that usually controls the episode isn’t producing the expected improvement, or the child keeps getting worse, it isn’t a good idea to simply keep waiting.

You need to contact the doctor promptly and, if there’s significant breathing difficulty, go to an emergency service.È necessario ricontattare rapidamente il medico e, in presenza di una difficoltà respiratoria importante, rivolgersi a un servizio di urgenza.

How is asthma treated?Come si cura l’asma?

Treatment depends on how often symptoms happen and how severe they are.

There are medications used to quickly relieve bronchial obstruction during a flare-up, and controller treatments that, when needed, are used regularly to reduce airway inflammation and prevent new episodes.

Not every child needs the same treatment.

A child with very occasional episodes has different needs from a child who frequently has coughing, wheezing, or limits on physical activity.

For this reason, treatment has to be set by the doctor and reassessed regularly over time.

When should we contact the pediatrician?Quando dobbiamo contattare il pediatra?

It’s worth talking to the pediatrician when a child has recurrent episodes of coughing and wheezing, when the cough consistently shows up at night or during physical activity, or when it tends to drag on for a long time after every respiratory infection.È opportuno parlarne con il pediatra quando il bambino presenta episodi ricorrenti di tosse e respiro sibilante, quando la tosse compare sistematicamente durante la notte o durante l’attività fisica oppure quando tende a protrarsi a lungo dopo ogni infezione respiratoria.

An evaluation is especially important when there’s also a personal or family history of asthma, allergies, or atopic dermatitis.

It isn’t the parent’s job to decide whether these symptoms are caused by asthma.

Parents need to observe them, notice when they recur, and report them.

When do we need to act quickly?Quando invece dobbiamo muoverci rapidamente?

Significant breathing difficulty is a different situation from a simple cough.

is visibly struggling to breathe, is using the chest muscles intensely, can’t talk normally, has to stay sitting up to breathe, appears progressively more exhausted, or isn’t responding to a treatment previously prescribed. If breathing is significantly compromised, go to the Emergency Room right away.respira con evidente fatica, utilizza intensamente i muscoli respiratori, non riesce a parlare normalmente, deve rimanere seduto per respirare, appare progressivamente affaticato oppure non risponde alla terapia precedentemente prescritta.

When a parent tells me:Quando un genitore mi dice:

“My child coughs every night, could it be asthma?”“Mio figlio tossisce tutte le notti: sarà asma?”

the answer can’t be given simply by the fact that there’s a cough.la risposta non può essere data semplicemente dalla presenza della tosse.

I want to know when it happens, for how long, whether it comes on with running, whether it was preceded by a cold, whether there’s wheezing, whether there are allergies, and above all, how the child is breathing.

Sometimes this information is enough to point us in the right direction.

Other times, the child needs to be examined.Altre volte è necessario visitarlo.

So the important message for parents is simple:Il messaggio importante per il genitore è quindi semplice:

you don’t have to diagnose asthma. You have to observe the child.non dovete diagnosticare l’asma. Dovete osservare il bambino.

A recurrent cough is worth mentioning to the pediatrician.

Breathing difficulty, on the other hand, especially if it’s steadily getting worse, shouldn’t be underestimated or simply waited out.

And it’s exactly this distinction, between a symptom to observe and breathing difficulty that calls for prompt action, that can matter most for a child’s safety.
