“Doctor, my child doesn’t eat.”“Dottore, mio figlio non mangia.”
“Doctor, I’m afraid he isn’t growing enough.”“Dottore, ho paura che non cresca abbastanza.”
These are among the concerns parents bring to me most often.
But judging how much a child really eats isn’t simple. Parents can get the impression their child eats very little, especially when comparing the amounts to those of an adult, a sibling, or another child.
So when I’m told a child eats little, one of the first things I want to understand is: how is he growing?
A child can eat a lot one day and very little the next. He might have a big lunch and barely touch dinner.
That’s why it’s hard to judge a child’s nutritional status simply by looking at what’s left on the plate.
The pediatrician has a far more useful tool: how growth is trending over time. Weight and height are plotted on growth charts and compared with previous measurements.
Not every child needs to sit at the fiftieth percentile. Some children are constitutionally small, others constitutionally big. What matters most is how that particular child’s growth is progressing over time.
Very often a worried parent arrives convinced their child eats far too little, and looking at the growth curve is exactly what reassures them.
After the first year, it can suddenly seem like he’s eating lessDopo il primo anno sembra che improvvisamente mangi meno
This is an extremely common situation.È una situazione estremamente comune.
During the first year of life, growth is very rapid. A baby weighing just over 3 kg (6.5 lb) at birth can reach around 9 to 10 kg (20 to 22 lb) by age one: in twelve months, they’ve nearly tripled their weight.
In the second year, the pace changes considerably. A child of about 10 kg at one year might reach roughly 12 to 13 kg by age two.
It’s understandable, then, that nutritional needs tied to growth also change. Meanwhile, that baby who used to spend so much time still has started walking, running, climbing, exploring, and playing constantly.È quindi comprensibile che cambino anche le esigenze alimentari legate alla crescita.
The relationship between growth, physical activity, and energy needs has completely changed. So an apparent drop in appetite after the first year can be entirely physiological.
During the first months, a baby’s diet is mostly liquid. With the introduction of solid foods, what changes isn’t just what he eats, but also how parents perceive the quantities.
We can’t simply compare the volume of milk taken in the early months with the volume of food we later find on the plate.
It’s precisely at this stage that a very common mistake can creep in: thinking the child necessarily has to eat a certain set amount.È proprio in questa fase che può nascere un errore molto comune: pensare che il bambino debba necessariamente mangiare una determinata quantità.
That’s when the encouraging, the negotiating, the distractions, and sometimes the conflicts begin.
At the table, we divide the rolesA tavola dividiamo i compiti
I find it very useful to explain to parents that adult and child have different jobs.
The parent decides what, when, and how to offer food; the child decides whether to eat, and how much, from what’s offered.
This doesn’t mean leaving the child to build their own diet entirely on their own. It’s the parent’s job to provide good-quality, appropriate food.
But once the meal has been offered, there shouldn’t be a need to chase the child with the spoon or force them to finish a set amount.
What should we offer?Che cosa dobbiamo offrire?
The goal isn’t to build a perfectly measured meal every single day, but to offer a varied, balanced diet over time.L’obiettivo non è costruire ogni giorno un pasto perfetto al grammo, ma offrire nel tempo un’alimentazione varia ed equilibrata.
This should include adequate protein sources, mostly complex carbohydrates, age-appropriate fats, fruit and vegetables, and sources of calcium.
Iron deserves particular attention, especially after the start of complementary feeding. Meat and fish are important sources of well-absorbed iron; other foods also naturally contribute to overall intake.
Fruit and vegetables provide vitamins and other micronutrients, and vitamin C also helps absorb the non-heme iron found in plant foods. In the early years, it’s best to especially limit added sugars and overly salty foods.
Breast milk and iron: both matterLatte materno e ferro: entrambi importanti
In the first months of life, breast milk is the ideal food, and breastfeeding can be on demand.
In the second half of the first year, breastfeeding can naturally continue, but it needs to be gradually accompanied by complementary feeding. At this stage, ensuring adequate iron intake becomes particularly important.
We don’t need to choose between breast milk and solid food: both matter, and they should progressively work together.
A significant, prolonged iron deficiency can affect a child’s health and development. Low iron can also increase intestinal absorption of certain toxic metals, such as lead.
This is why, once complementary feeding begins, it’s important to regularly offer foods that contribute good iron intake as well.
How much milk?Quanto latte?
It can help to give parents a simple ballpark figure. In the second half of the first year, we can think roughly in terms of about half a liter of milk a day.
After age one, the amount can gradually decrease, reaching roughly around 250 ml (about a cup) a day.
These amounts aren’t a mandatory target or a strict limit. They’re simply reference points that help show how milk’s role changes as solid foods increase.
Part of the nutrition milk provides can also come from other dairy, like yogurt and cheese, while water covers adequate hydration. For a child still breastfeeding, of course, it makes no sense to try to measure feeds in milliliters.
The important idea is to avoid letting excessive amounts of milk crowd out appetite and room for other foods, especially those that matter for iron intake.
“He always eats the same things”“Mangia sempre le stesse cose”
Between ages 2 and 4, many children go through periods of notable food selectivity. They always want the same pasta, the same type of meat or fish, the same fruit.
Within certain limits, this repetition doesn’t necessarily need to worry us. I don’t consider it essential that a child eat pasta today, rice tomorrow, and bread the day after. What matters to me most is that the main food groups are represented over the course of the day and the week.
Variety should keep being offered, letting the child get to know different flavors, but without turning it into a requirement.
When possible, we eat what the family eatsMangiamo, quando possibile, quello che mangia la famiglia
I find it helpful for a child to gradually start eating the same foods as the rest of the family, naturally adapted to their age.
This lets them discover different flavors and learn by watching and imitating their parents.
In younger children, particular attention should also go to the shape, size, and texture of food, which need to match their chewing and swallowing abilities to reduce the risk of choking.
Firmer foods should therefore be prepared and cut appropriately for the child’s age, avoiding dangerous shapes and sizes.
Being flexible doesn’t mean cooking to orderEssere flessibili non significa cucinare su ordinazione
Of course, we can respect a child’s tastes. If he prefers plain pasta over pasta with tomato sauce, that’s not a problem. If she likes meat prepared a certain way, or a particular kind of fruit, we can certainly take that into account.
But there’s a difference between respecting tastes and systematically preparing a completely different meal every time the child refuses the family’s meal.
There’s also an emotional reason. If a mother or father made a dish specifically for the child and it gets refused, it’s understandable they might feel more invested and push back: “I made this just for you: at least try it.”C’è anche una ragione emotiva.
When what’s offered is simply part of the family meal, on the other hand, even a refusal carries less emotional weight.
Let’s not turn mealtime into a battleNon trasformiamo il pasto in una battaglia
“One more bite.” “Finish everything.” “If you eat this, you can have dessert.”“Ancora un boccone.” “Finisci tutto.”
These are strategies that almost always come from worrying that the child isn’t eating enough. But they can gradually turn mealtime into a battleground between adult and child.
Forcing a child to eat can also interfere with their ability to recognize hunger and fullness.
The goal shouldn’t be an empty plate. It should be helping the child build a calm relationship with food.L’obiettivo non deve essere vedere il piatto vuoto.
No tablets or phones to get them to eatNiente tablet o telefono per farlo mangiare
Using a screen during meals can seem like an effective solution. The child watches the video, gets distracted, and meanwhile the parent manages to get a few more spoonfuls in.Utilizzare uno schermo durante il pasto può sembrare una soluzione efficace.
But that’s exactly the problem. If we’ve established that we shouldn’t force a child to eat, we shouldn’t do it by distracting them either.
A child should take part in the meal consciously, and gradually learn to recognize their own hunger and fullness.
Eating together also means learningMangiare insieme significa anche imparare
A meal isn’t only there to provide calories and nutrients. It’s also a moment of family life and learning.
The child watches the parents and imitates them: how they use cutlery, how they taste food, how they behave at the table.
He listens to conversations, learns new words and expressions, tells his own stories, and gradually learns how to have a conversation and interact with adults.
A child absorbed in a tablet misses out on much of that. That’s why, whenever possible, I think eating together without screens really matters.
When a parent fears their child isn’t growing enough, they don’t need to try to interpret percentiles and growth curves on their own. That’s the pediatrician’s job.
The parent’s job is simpler, and at the same time essential: observe your child and report what you see.
If a mother or father has the impression the child is eating far too little or isn’t growing properly, it’s right to talk to the pediatrician about it.Se una madre o un padre hanno l’impressione che il bambino mangi troppo poco o non stia crescendo adeguatamente, è corretto parlarne con il pediatra.
What other symptoms should we watch for?Quali altri sintomi dobbiamo osservare?
Extra attention is warranted when poor appetite or a sense that growth is insufficient comes together with other symptoms, such as:La preoccupazione merita particolare attenzione quando allo scarso appetito o all’impressione di una crescita insufficiente si associano altri sintomi, come:
⚠ Mention these to your pediatrician too:Segnalate anche questi al pediatra:
- recurrent vomitingvomito ricorrente
- persistent diarrheadiarrea persistente
- palenesspallore
- tiredness or easy fatiguestanchezza o facile affaticabilità
- irritability or significant changes in behaviorirritabilità o cambiamenti significativi del comportamento
- persistent sleep difficultiesdifficoltà persistenti del sonno
- other symptoms unusual for the childaltri sintomi insoliti rispetto alle condizioni abituali del bambino
This information is valuable because it can help the pediatrician understand whether there’s an underlying issue worth looking into more closely behind an apparently poor appetite.
Sometimes the problem isn’t on the plateA volte il problema non è nel piatto
An interesting example is a child who habitually snores at night. Significant breathing obstruction during sleep, for instance with enlarged adenoids, can force the child to work harder to breathe and deeply disrupt sleep quality.
In the more significant cases, this can also come with difficulty growing. That’s why we shouldn’t focus all our attention exclusively on food.
When a parent tells me “My child isn’t growing,” it can be just as important to know that “he snores every night,” “she’s become more tired,” “he looks pale,” “she often has diarrhea,” or “he sleeps very poorly.”Quando un genitore mi dice: “Mio figlio non cresce” può essere altrettanto importante sapere che: “Russa tutte le notti”, “è diventato più stanco”, “è pallido”, “ha spesso diarrea” oppure “dorme molto male”.
The pediatrician will assess growthSarà il pediatra a valutare la crescita
The pediatrician will reconstruct how weight and height have progressed over time, comparing current measurements with previous ones and evaluating them in the context of that child’s own characteristics.
A child who has always grown along their own trajectory is very different from a child whose growth pattern is progressively and significantly changing.
If a real problem does emerge, it will be up to the pediatrician, based on the child’s history and the exam, to decide whether further tests are needed and which ones.
Parents don’t need to diagnose a growth problem. They need to notice that something might have changed, and say so.Il genitore non deve quindi diagnosticare un problema della crescita.
When a mother tells me, “Doctor, my child eats nothing,” very often I find that the child is, in fact, growing perfectly.
The problem isn’t necessarily how much the child eats. It can be the amount we adults think they should eat.
Parents need to focus on providing good-quality, appropriate food, creating a calm atmosphere, and letting the child take part in the family meal. The child needs to be able to gradually learn to recognize their own hunger and fullness.
Of course, there are children who really do grow poorly, and situations where a poor appetite is a sign of an underlying problem. But in that case, it won’t be the half-full plate that gives us the answer.
It will be the child’s history, any associated symptoms, the exam, and how growth has progressed over time.
When the child is well and growing regularly, very often the best thing we can do is: offer good food, sit at the table with them, turn off the screens, and let them have the freedom to decide how much to eat.
