Fever is one of the most common reasons parents contact their pediatrician, and understandably one of the symptoms that causes the most worry.
When a parent calls me and says, “Doctor, my child has a fever,” the number on the thermometer is only one of the things I want to know.
The first questions are others: How old is the child? How long has the fever lasted? Are there other symptoms? And above all: how is the child doing?
What do we mean by fever?Che cosa intendiamo per febbre?
In general, in children we speak of fever when body temperature reaches or exceeds 38°C (100.4°F).In generale, nel bambino si parla di febbre quando la temperatura corporea raggiunge o supera i 38 °C.
It’s also important to know how it was measured. There are different measurement methods, and they don’t all have the same accuracy.È importante anche sapere come è stata misurata.
In very young children, rectal temperature gives an accurate reading. Armpit temperature is very simple and practical, but can be less precise. Ear thermometers can also be used, especially in older children, but the result depends a lot on correct positioning of the device.
So when you contact your pediatrician, don’t just say “It’s 38.5.” Also mention how you measured the temperature.
The first thing to consider is ageLa prima cosa da considerare è l’età
A fever in the first three months of life should always be taken seriously and requires prompt medical evaluation.
A temperature of 38°C in a baby a few weeks old should not be treated the same way as the same temperature in a five-year-old who is playing and feels fine.
That’s why one of the first questions I always ask is: “How many months or years old is the child?”Per questo una delle prime domande che faccio è sempre: “Quanti mesi o quanti anni ha il bambino?”
How long has the fever lasted?Da quanto tempo ha la febbre?
A fever that started a few hours ago is different from one that has continued for several days. I also want to know how it has behaved over time.
One situation worth watching closely is when fever appears at the start of an illness, then goes away, and after a few days without fever, comes back. In this case, it’s a good idea to have the child reassessed, since a complication or secondary infection may have occurred.
Look at the child, not just the thermometerGuardiamo il bambino, non soltanto il termometro
This is probably one of the most important things I try to explain to parents.
A child with 39°C who, once the temperature comes down, becomes lively again, interacts, drinks, and maybe starts playing can be much less worrying than a child with a lower temperature who looks listless, unresponsive, or very unwell.
That’s why I ask: Is he drinking? Is she urinating normally? Is he interacting? Is she playing? Is he easily consoled? How is she doing when the temperature comes down?Per questo chiedo: beve? Urina normalmente? Interagisce? Gioca? È facilmente consolabile? Come sta quando la temperatura scende?
The temperature is a number. The patient is the child.La temperatura è un numero. Il paziente è il bambino.
What other symptoms are present?Quali altri sintomi sono presenti?
It’s important to know whether, along with the fever, the child also has:È importante sapere se insieme alla febbre sono comparsi:
- a runny nose or coughraffreddore o tosse
- a sore throatmal di gola
- vomiting or diarrheavomito o diarrea
- paindolore
- difficulty breathingdifficoltà respiratoria
- a rash or spots on the skineruzioni o macchie sulla pelle
- other changes from the child’s usual behavioraltri cambiamenti rispetto al comportamento abituale del bambino
A runny nose and cough, especially in a child who is otherwise doing well, very often go along with common childhood viral infections. An isolated, persistent fever, on the other hand, generally deserves a few more questions.
What if a rash appears?Se compaiono delle macchie sulla pelle?
A rash that appears during a fever should be reported to the pediatrician. It’s not up to the parent to decide alone whether the rash is worrying or not.
It’s helpful to describe it and, when possible, show it to the pediatrician. The doctor will interpret it together with the child’s age, how long the fever has lasted, and the other symptoms.È utile descriverla e, quando possibile e appropriato, mostrarla al pediatra.
Is fever always the enemy?La febbre è sempre un nemico?
No. Fever is a natural response of the body to infection, and it’s part of the mechanisms the body uses during the immune response. That’s why we don’t necessarily need to try to eliminate every rise in temperature. The goal isn’t to constantly bring the child back to 36.5°C.No.
Let’s not try to “beat the thermometer”Non cerchiamo di “avere la meglio sul termometro”
It’s easy to fall into a kind of race against the temperature: “It was 39, I gave the fever medicine, and now it’s still 38. Devo dargli qualcos’altro?”
In reality, fever medicine should mainly be used when the child is uncomfortable or unwell, not simply to normalize the number on the thermometer. If the child is generally in good condition, there’s no need to constantly chase the temperature trying to bring it down at all costs.
We look at the child, we don’t chase the thermometer.Guardiamo il bambino, non inseguiamo il termometro.
Acetaminophen or ibuprofen?Paracetamolo o ibuprofene?
Both acetaminophen (paracetamol) and ibuprofen can be used in children, when appropriate, to ease the discomfort that comes with fever. The choice shouldn’t simply depend on which of the two brings the temperature down more.
Ibuprofen requires particular caution, for example, when the child is dehydrated or has significant vomiting or diarrhea. The dose must be right for the child’s weight, and the intervals between doses must be respected.L’ibuprofene richiede particolare cautela, per esempio, quando il bambino è disidratato o presenta vomito o diarrea importanti.
It’s not advisable to give acetaminophen and ibuprofen at the same time just to bring the temperature down further.
Watch out: some medicines already contain a fever reducerAttenzione: alcuni farmaci contengono già un antipiretico
Some combination products, used for example for cold symptoms or pain, may already contain acetaminophen or another pain/fever reducer. If you then give another medicine without checking its composition, you risk unintentionally giving the same active ingredient twice.
It’s always important to read the composition of any medicine your child is taking, and when in doubt, ask your pediatrician or pharmacist. We shouldn’t just count how many medicines we’ve given: we need to know what’s in them.È sempre importante leggere la composizione dei medicinali che il bambino sta assumendo e, in caso di dubbio, chiedere al pediatra o al farmacista.
What about aspirin?L’aspirina?
Aspirin should not be used as a routine fever reducer in children. Acetylsalicylic acid, particularly in association with certain viral infections, has been linked to Reye’s syndrome, a rare but potentially very serious condition.L’aspirina non deve essere utilizzata come comune antipiretico nel bambino.
Aspirin can be prescribed in children for specific conditions under medical supervision, but it should not be used independently by parents to treat fever.L’aspirina può essere prescritta in età pediatrica per particolari malattie e sotto controllo medico, ma non deve essere utilizzata autonomamente dai genitori per trattare la febbre.
What about febrile seizures?E le convulsioni febbrili?
Febrile seizures mainly affect young children and often tend to occur in the early stages of a fever episode. Sometimes the seizure is actually the first sign that alerts parents that the child is developing a fever.
It’s important to know that continuously trying to keep the temperature normal with fever medicine does not guarantee prevention of febrile seizures.
Let’s have them drinkFacciamolo bere
During a fever, a child can lose more fluids and become dehydrated more easily. It’s important to offer fluids frequently. There’s no need to force large amounts all at once: small amounts, offered often, work well. For a breastfed baby, you can continue to offer the breast.
Good hydration is one of the simplest and most important things we can do during a fever.
Let’s let them restLasciamolo riposare
If the child is sleepy, let him sleep. If she wants to stay quiet on the bed or the sofa, let her rest. There’s no need to keep him awake or constantly stimulate him to check on his condition.
Of course, a child who is sleeping because she is tired is different from a child who is unusually drowsy, hard to wake, or unresponsive: in that case, medical evaluation is needed. When the fever comes down and the child spontaneously goes back to playing, this is generally a reassuring sign.
How should I dress them?Come vestirlo?
When the temperature is rising, the child may feel chilly and shivery and may want to be covered. Afterward, it’s important to avoid both overdressing and undressing the child to try to cool them down.
The goal is simply to keep them comfortable, dressed appropriately for the room temperature. There’s no need for cold baths or sponging to try to bring the fever down.L’obiettivo è semplicemente mantenerlo comodo, con un abbigliamento adeguato alla temperatura dell’ambiente.
When should the child be seen quickly?Quando deve essere visitato rapidamente?
⚠ Contact the pediatrician promptly when:Contattate rapidamente il pediatra quando:
- the fever appears in the first months of life, especially under 3 monthsla febbre compare nei primi mesi di vita, in particolare sotto i 3 mesi
- the child looks very unwell, unresponsive, or is hard to wakeil bambino appare molto abbattuto, poco reattivo o difficilmente risvegliabile
- there is difficulty breathingpresenta difficoltà respiratoria
- the child is drinking very little or shows signs of dehydrationbeve molto poco o mostra segni di disidratazione
- the child is urinating much less than usualurina molto meno del solito
- a seizure occurspresenta una convulsione
- a rash appears during the fever, especially spots that don’t fade temporarily under pressurecompare un’eruzione cutanea durante la febbre, soprattutto se le macchie non scompaiono temporaneamente alla pressione
- there is significant or persistent painpresenta dolore importante o persistente
- the child’s overall condition is worseningle condizioni generali peggiorano
- the fever lasts for several daysla febbre persiste per diversi giorni
- the fever comes back after having gone awaydopo essere scomparsa, la febbre ricompare
And there’s another criterion I consider important: if, as a parent, you look at your child and feel that something isn’t right, it’s worth contacting the pediatrician.E c’è un altro criterio che considero importante: se i genitori guardano il proprio bambino e sentono che qualcosa non va come al solito, vale la pena contattare il pediatra.
Can they go to school or daycare?Può andare a scuola o all’asilo?
A child with a fever should stay home. For common viral illnesses, as a practical rule, it’s a good idea to wait at least 24 hours without fever and without needing fever medicine before returning to school or daycare.
Of course, some infectious illnesses call for different exclusion periods, and the specific guidance for those should be followed. It’s also not enough for the thermometer to read normal: the child needs to be well enough again to comfortably get through a normal day.
After many years in pediatrics, when a parent tells me, “Doctor, my child has a fever of 39,” I don’t just think about that number.
I want to know how old the child is, how long the fever has lasted, what other symptoms are present, whether he’s drinking, whether she’s urinating, and above all how he behaves when the temperature comes down.
Very often, it’s exactly this information, combined with observing the child, that helps us understand whether we can safely keep waiting, or whether it’s time for a visit.
Fever, in most cases, is part of the body’s response to illness. We shouldn’t try to beat the thermometer. We need to understand what’s happening with the child.
