Skin Rashes in Children: What Should Parents Watch For?

Rash-related illnesses are, in a way, part of pediatric history.

When I started out as a pediatrician, some illnesses had such distinctive patterns that it was often possible to recognize them simply by watching the child and following how the rash evolved.

There was measles, with fever, cough and conjunctivitis followed by the rash; rubella; chickenpox, with its characteristic lesions evolving from papules to blisters and then to crusts; and scarlet fever, which we still see today, with its rash that feels finely rough to the touch.C’era il morbillo, con febbre, tosse e congiuntivite seguite dalla comparsa dell’esantema; la rosolia; la varicella, con le caratteristiche lesioni che evolvevano da papule a vescicole e successivamente a croste; e la scarlattina, che vediamo ancora oggi, con il suo caratteristico esantema finemente ruvido al tatto.

As vaccination has spread, some of the classic rash illnesses have become far less common.

But for a parent, the question has stayed the same:Ma per un genitore la domanda è rimasta la stessa:

“Some marks have appeared on my child’s skin. Should I be worried?”“Sono comparse delle macchie sulla pelle di mio figlio. Devo preoccuparmi?”

The first thing to know is that not all rashes are alike, and not all of them are caused by an infection.

We don’t just look at the marksNon guardiamo soltanto le macchie

When a parent contacts me about a rash, I’m not only interested in seeing what the lesions look like.

I try to reconstruct what happened around the time it appeared.

How old is the child? Is there fever? Did the marks appear before, during, or after the fever? Where did they start, on the face, the trunk, the hands or the feet? Sul volto, sul tronco, sulle mani o sui piedi?

Are they flat or raised? Do they itch? Are there blisters? Do they fade when you gently stretch or press the skin? How are they spreading?Sono piatte oppure rilevate? Prudono?

And above all:E soprattutto:

how is the child doing?come sta il bambino?

How the rash looks matters, but it always has to be read together with the child’s overall condition.L’aspetto dell’eruzione è importante, ma deve sempre essere interpretato insieme alle condizioni generali.

The sequence of events matters tooAnche la sequenza degli eventi è importante

Some illnesses follow a particularly distinctive course.Alcune malattie presentano un’evoluzione particolarmente caratteristica.

In roseola, common especially in young children, we can see several days of high fever. When the fever goes away and the child seems to feel better, the rash appears, mostly on the trunk.

The sequence:La sequenza:

fever → fever goes away → rash appearsfebbre → scomparsa della febbre → comparsa dell’esantema

can be very telling.può essere molto indicativa.

In fifth disease, caused by parvovirus B19, we can initially see the characteristic redness of the cheeks, the so-called “slapped cheek” look. Afterward, a lace-like rash can appear on the limbs, which can persist or come and go for some time and become more noticeable with heat.

In hand-foot-and-mouth disease, it’s especially useful to look at where the lesions are: mouth, hands and feet, sometimes also the limbs and buttocks. There can also be blisters at the back of the throat, as happens in herpangina.

The principle to remember is simple:Il principio da ricordare è semplice:

it isn’t just what a mark looks like that matters, but also where it appeared and at what point in the illness.non conta soltanto come è fatta una macchia, ma anche dove è comparsa e in quale momento della malattia.

A simple test: does the mark fade under pressure?

When there are reddish patches, a simple observation can give important information.

If you gently stretch the skin or press on it lightly, an ordinary redness (erythema) tends to fade or briefly disappear.

A petechia or purpura, on the other hand, does not fade under pressure.

This simple check doesn’t let a parent make a diagnosis, of course, but it gives an important piece of information to pass on to the pediatrician.

In particular, lesions that don’t fade under pressure, combined with fever and a child who appears very unwell or in poor general condition, need urgent evaluation.

Even in a child who otherwise seems well, the sudden appearance of numerous petechiae or bruises should still be reported to the pediatrician.

We look at where the lesions are, but also where they aren’tGuardiamo dove sono le lesioni, ma anche dove non ci sono

How a rash is distributed can be very informative.

A simple example is heat rash, common during warm periods. The small lesions tend to affect mainly the areas where the child sweats the most and that stay covered by clothing, such as the trunk.

But the areas that are spared can also give us information.

Atopic dermatitis and the diaper areaLa dermatite atopica e la zona del pannolino

In a young child with atopic dermatitis, we can see dry skin, redness and itching, while the area covered by the diaper is frequently relatively spared.

Under the diaper, a more humid environment forms that limits water loss from the skin and helps keep it better hydrated.

The diaper area shows us, very intuitively, just how important it is to keep the skin hydrated in atopic dermatitis.

Moisturizing isn’t a minor extra step: it’s a core part of managing the condition day to day.L’idratazione non rappresenta un semplice trattamento accessorio: è una parte fondamentale della gestione quotidiana della malattia.

Seborrheic dermatitis, which can sometimes be mistaken for atopic dermatitis, tends instead to favor skin folds and areas rich in oil glands.

Itching gives us informationIl prurito ci dà informazioni

“Does your child scratch?”“Il bambino si gratta?”

Itching can help us a lot.Il prurito può aiutarci molto.

Raised welts that are fleeting and intensely itchy can point toward hives.Pomfi rilevati, fugaci e intensamente pruriginosi possono orientare verso un’orticaria.

Dry skin with itchy, eczema-like lesions can be characteristic of atopic dermatitis.

Very intense itching, especially at night, should also raise the possibility of scabies.

Scabies: we ask about other family members tooScabbia: chiediamo anche degli altri familiari

When I suspect scabies, one of the first questions I ask is:Quando sospetto una scabbia, una delle prime domande che faccio è:

“Is anyone else in the family starting to itch?”“C’è qualcun altro in famiglia che ha cominciato a prudere?”

Scabies is highly contagious, and when it’s present within a household, it’s common for more than one family member to have itching or skin changes, even if not necessarily at the same time.

No other family members having symptoms doesn’t rule out scabies, but the same problem showing up in several people in the household is a very important clue.L’assenza di altri familiari con sintomi non esclude la scabbia, ma la presenza dello stesso problema in più persone della famiglia rappresenta un elemento anamnestico molto importante.

The pediatrician will then assess where the lesions are, signs of scratching, and any characteristic burrows in the skin.

Hives are common in children and can appear suddenly.L’orticaria è frequente nei bambini e può comparire improvvisamente.

Often it isn’t possible to pin down the cause with certainty. It can be linked to foods or other factors, but in children it can also appear during a viral infection.

The lesions are generally raised above the skin surface, itchy, and above all have one key feature:Le lesioni sono generalmente rilevate rispetto alla superficie cutanea, sono pruriginose e hanno soprattutto una caratteristica:

they’re fleeting.sono fugaci.

A single welt tends to fade, and new lesions can appear in different spots.Il singolo pomfo tende a scomparire e altre lesioni possono comparire in zone differenti.

This means that if you photograph a given area today and photograph it again later, the individual hives lesions shouldn’t still be exactly the same, in exactly the same spot.

The rash may still be there, but the individual welts have changed.L’eruzione può esserci ancora, ma i pomfi sono cambiati.

This feature can help the pediatrician tell it apart from other conditions, like erythema multiforme, where individual lesions instead stay fixed for longer.

So we shouldn’t only ask ourselves:Non dobbiamo quindi chiederci soltanto:

“What does this mark look like?”“Com’è fatta questa macchia?”

but also:ma anche:

“How long does it stay in the same spot?”“Quanto tempo rimane nello stesso punto?”

Hives during antibiotic treatment: does it mean allergy?Orticaria durante un antibiotico: significa allergia?

Not necessarily.Non necessariamente.

A rash appearing while a child is taking an antibiotic doesn’t automatically prove they’re allergic to that medication.

The infection itself, often viral, can be responsible for the rash. On top of that, liquid formulations contain other ingredients besides the active drug.

We need to carefully reconstruct the sequence of events: when the medication was taken, how long afterward the rash appeared, what it looks like, and what other symptoms are present.

It’s important not to too readily label a child as “allergic to that antibiotic,” because that label can follow them for many years and, in the future, prevent them from using a drug that might actually be the right choice.È importante non attribuire troppo facilmente a un bambino l’etichetta di “allergico a quell’antibiotico”, perché potrebbe accompagnarlo per molti anni e impedirgli in futuro di utilizzare un farmaco che potrebbe invece essere appropriato.

Of course, a genuine allergic reaction to an antibiotic is possible and must be taken seriously.

So if a rash appears during antibiotic treatment, photograph it and contact the pediatrician.

When hives become an emergencyQuando l’orticaria diventa urgente

A child with welts and itching who is breathing normally and otherwise well is in a very different situation from a child whose rash is suddenly accompanied by breathing difficulty, a change in voice, significant swelling of the tongue or throat, repeated vomiting, marked weakness, or loss of consciousness.

⚠ Seek urgent evaluation if hives are accompanied by:Richiedete una valutazione urgente se all’orticaria si associano:

  • breathing difficultydifficoltà respiratoria
  • a change in the voicealterazione della voce
  • significant swelling of the tongue or throatgonfiore importante della lingua o della gola
  • repeated vomitingvomito ripetuto
  • marked weakness or loss of consciousnessmarcata debolezza o perdita di coscienza

These signs can point to a systemic allergic reaction and need to be evaluated right away.

A photo can help a lotUna fotografia può aiutare molto

Today it’s common for a parent to send me a photo and ask:Oggi è frequente che un genitore mi invii una fotografia chiedendomi:

“Doctor, what are these marks?”“Dottore, che cosa sono queste macchie?”

A photo can be extremely useful, but I’d recommend taking two, if possible.

One close-up, to clearly show the features of a single lesion.

A second one from further away, to show how the lesions are distributed on the body and which areas are spared.

If the rash changes quickly, as can happen with hives, it can help to photograph the same area again a few hours later.

But a photo doesn’t tell the whole story.Ma una fotografia non racconta tutto.

It doesn’t tell me when the fever started, whether the child is scratching, what other symptoms are present, and above all, how the child is doing.

That’s why a photo is far more useful when it comes together with a few precise pieces of information.

Fever and rash: when to ask for help quicklyFebbre ed eruzione cutanea: quando chiedere rapidamente aiuto?

A rash that appears during a fever is always worth mentioning to the pediatrician.

Most of the time it will turn out to be a benign condition, often linked to a viral infection.

A prompt evaluation is needed, though, especially when:Bisogna però richiedere una valutazione rapida soprattutto quando:

⚠ Contact the pediatrician promptly if:Contattate tempestivamente il pediatra se:

  • the lesions don’t fade under pressure, especially if the child has fever and appears unwellle lesioni non scompaiono alla pressione, in particolare se il bambino ha febbre ed è sofferente
  • the child appears very listless, pale, drowsy, or poorly responsiveil bambino appare molto abbattuto, pallido, sonnolento o poco reattivo
  • there’s breathing difficultypresenta difficoltà respiratoria
  • there’s significant swelling of the lips, tongue, or throatcompare gonfiore importante di labbra, lingua o gola
  • there’s a severe headache or repeated vomitingsono presenti cefalea importante o vomito ripetuto
  • numerous petechiae, purpura, or bruises appear without an obvious explanationcompaiono numerose petecchie, porpora o ecchimosi senza una spiegazione evidente
  • there’s a rapid worsening of the child’s overall conditionsi verifica un rapido peggioramento delle condizioni generali

In these situations, sending a photo and waiting isn’t enough: the child needs to be seen promptly.

What should parents watch for?Che cosa devono osservare i genitori?

When marks appear on your child’s skin, you don’t necessarily need to try to name the illness right away.

Instead, observe:Osservate invece:

  • when they appearedquando sono comparse
  • where they appeared firstdove sono comparse per prime
  • how they’re spreadingcome si stanno diffondendo
  • whether they’re flat or raisedse sono piatte o rilevate
  • whether they itchse prudono
  • whether there are blistersse sono presenti vescicole
  • whether they fade under pressurese scompaiono alla pressione
  • whether they change position over the hoursse cambiano posizione nel corso delle ore
  • whether they were preceded or accompanied by feverse sono state precedute o accompagnate dalla febbre
  • what other symptoms are presentquali altri sintomi sono presenti
  • whether other family members have similar symptomsse altre persone della famiglia presentano manifestazioni simili

And above all:E soprattutto:

watch how the child is doing.osservate come sta il bambino.

Something I’ve learned from pediatric practiceUna cosa che ho imparato dalla pratica pediatrica

Faced with a rash, how the lesions look matters, but it rarely tells the whole story on its own.

Where it is, how it’s distributed, the itching, how it changes over time, its relationship with fever, and the child’s overall condition are often just as important as the picture itself.

A parent doesn’t need to be able to recognize fifth disease, hives, or atopic dermatitis on their own. It’s far more useful to know what to observe.

It will then be up to the pediatrician to put these pieces together, reassure the family when possible, and understand when the child instead needs to be seen directly or needs further tests.
