Abdominal Pain in Children: What Should Parents Watch For?

Mal di pancia nel bambino - cover

Abdominal pain is one of the most common situations in childhood. In the vast majority of cases it has a benign cause and a favorable course. But precisely because tummy aches are so common, we need to be careful not to overlook the much less frequent situations where pain can be the first sign of something more serious.

When a parent tells me “Doctor, my child has a tummy ache,” it’s not enough to know the pain exists. I want to know how old the child is, where it hurts, for how long, what kind of pain it is, how it changes over time, what other symptoms are present, and above all how the child is doing.

Acute pain or pain that keeps coming back? And why the child’s age mattersDolore acuto o dolore che si ripete nel tempo? E l’età del bambino

A first important distinction is duration: we can have acute abdominal pain, present for a few hours or days, or chronic or recurrent pain that persists or tends to come back over time. These are different situations and call for different reasoning.

As with many other pediatric symptoms, age is essential: the possible causes of abdominal pain change across the stages of growth, and pain in an infant needs to be interpreted differently from the same symptom in an eight-year-old or a teenager. That’s why age is one of the first things I always ask about.

What kind of pain is it, where does it hurt, and let’s not forget constipationChe tipo di dolore è, dove fa male, e non dimentichiamoci della stitichezza

Let’s try to understand how the pain presents. Is it crampy, coming and going, or dull, constant and persistent? Does the child always point to the same spot, or does the pain move around? Does it relate to meals, come before a bowel movement, improve afterward? Does he keep playing and moving normally, or does he prefer to stay still because movement makes it worse? What matters isn’t just how much it hurts — it’s also how the pain behaves over time.

Location also gives us useful information. Pain that tends to settle in the lower right of the abdomen deserves particular attention, especially when it becomes constant and comes with nausea, vomiting, low-grade fever, or a worsening general condition — one of the things to rule out in these cases is acute appendicitis. Not every pain in this area means appendicitis, of course, but it’s a location that deserves attention. Pain mainly in the lower left, on the other hand, is often linked to constipation.

Constipation, in fact, is a very common cause of abdominal pain in children. It’s useful to know when the child last had a bowel movement, whether the stool was hard, whether there was difficulty or pain passing it, and above all whether the tummy ache improves afterward — a simple question like “When did he last go?” can turn out to matter a great deal. When the history clearly fits with stool retention, the pediatrician can recommend treatment for the constipation and then check whether the pain improves or disappears afterward: sometimes that simple observation completely changes how we read the picture.

In acute gastroenteritis the pain is often crampy. A useful pattern to notice is that the pain often comes before a bowel movement and tends to ease, or even disappear, after loose or watery stools: the child may complain of a tummy ache, have a bout of diarrhea shortly after, and then feel noticeably better. A different picture is pain that continues even after a bowel movement, especially if it becomes progressively more intense or settles in one place.

Pain that comes and goes in young children: intussusceptionUn dolore che va e viene nel bambino piccolo: l’invaginazione intestinale

There’s a particular cause of abdominal pain worth remembering especially in the first years of life: intussusception, which occurs most often between about 6 months and 3 years of age. The child can suddenly develop very intense, crampy pain — crying, becoming agitated, and pulling the legs up toward the abdomen — then the pain seems to stop and a period of apparent well-being can follow. Later, though, the pain comes back.

This alternation between intense pain crises and apparently calm intervals is an important clue and shouldn’t necessarily be reassuring: as things progress, the truly comfortable intervals can shrink, and the child can look increasingly listless, drowsy, or poorly responsive. Vomiting and blood in the stool can also appear, but these signs can show up late — we shouldn’t wait for blood in the stool before asking for an evaluation. A young child with repeated episodes of very intense pain, separated by periods when he seems to feel better, needs to be evaluated promptly.

Look at the whole child, not just the bellyGuardiamo il bambino, non soltanto la pancia

Pain always needs to be read together with the other symptoms. It’s important to know whether there is:Il dolore deve sempre essere interpretato insieme agli altri sintomi. È importante sapere se sono presenti:

  • FeverFebbre
  • Nausea or vomitingNausea o vomito
  • DiarrheaDiarrea
  • Blood in the stoolSangue nelle feci
  • Significant bloating or abdominal distensionImportante meteorismo o distensione dell’addome
  • Pain when urinatingDolore durante la minzione
  • Reduced urine outputRiduzione della quantità di urine
  • Skin rashesEruzioni cutanee
  • PallorPallore
  • Refusal to eatRifiuto dell’alimentazione
  • Constipation or hard stoolsStitichezza o feci dure

Diet can also play a role: eating a lot of certain foods, especially those high in simple sugars, can encourage fermentation and bloating and contribute to the pain. But above all, we need to look at the child’s overall condition: a child who complains of a tummy ache but keeps playing, drinking, and moving normally is very different from a child who is pale, still, clearly unwell, refusing food, or changing his behavior because every movement causes pain.

The problem doesn’t always start in the bellyNon sempre il problema nasce dalla pancia

The history matters a great deal, and not just for the symptoms present now: abdominal pain can also appear after an injury, and this should always be mentioned to the doctor, because a significant abdominal injury can involve the internal organs.

A child can also report abdominal pain when the actual problem lies elsewhere. In boys, for instance, testicular torsion can present with abdominal pain, nausea, or vomiting, and it’s a condition that requires urgent evaluation. In girls, especially as puberty approaches and during adolescence, gynecological causes also need to be considered: sudden, severe pain can, for example, be due to ovarian torsion, which likewise requires prompt evaluation.

Sometimes time is what helps us understandA volte è il tempo ad aiutarci a capire

It isn’t always possible to reach an immediate diagnosis in a child with abdominal pain. If the child is otherwise well and there are no warning signs, it can sometimes help to watch how things evolve over a short period. I often tell parents: “Let’s watch him and check back in an hour or two.” This doesn’t just mean waiting — it means using that time to gather more information. Has the pain gotten better or worse? Has it stayed in the same place, or moved? Has he had a bowel movement? Has vomiting or fever appeared? Is he still moving normally, or does he seem increasingly listless? Sometimes it’s exactly how things evolve over the following hours that helps us understand what’s really going on.Non sempre davanti a un bambino con dolore addominale è possibile arrivare immediatamente a una diagnosi.

Waiting can be a reasonable choice, but it needs to be watchful waiting: if we’ve agreed to check back in an hour or two, but in the meantime the pain increases quickly, becomes constant or clearly localized, repeated vomiting or blood in the stool appears, the abdomen becomes significantly distended, or the child becomes pale, very listless, or poorly responsive, we shouldn’t wait for the planned check-in — the situation needs to be reassessed sooner.

And if the tummy ache continues or keeps coming back?E se il mal di pancia continua o ritorna nel tempo?

A different situation is a child who complains of a tummy ache persistently or repeatedly for at least two months. Chronic or recurrent abdominal pain mainly affects children from school age onward and can continue into adolescence. In most cases we don’t find an organic illness that explains the pain: it’s frequently functional abdominal pain.

This is a concept I consider particularly important: calling pain “functional” doesn’t mean at all that the child is making it up. The child genuinely feels pain. It means that, even without an organic illness to explain the symptom, how the gut works and how sensitive it is can produce real, painful symptoms. Emotional and environmental factors — school difficulties, relationships with classmates, family tension, or other stressful periods — can encourage or amplify the pain, but that doesn’t make it any less real. That’s why the child needs to be listened to, and the pain taken seriously.

Functional pain is still real pain.Il dolore funzionale è comunque dolore vero.

With a child who has recurring tummy aches, I don’t just ask “Where does it hurt?” but also “When does it happen?” Does it show up mostly on school days? Before certain activities? Have there been changes in his life recently? How are things going with friends? Diet is worth looking at too: eating a lot of simple sugars can encourage fermentation and bloating and contribute to the symptoms — but before starting restrictive diets or cutting out certain foods on your own, it’s best to discuss it with the pediatrician first.

When does chronic pain need further investigation?Quando il dolore cronico deve essere approfondito?

Even though most recurrent abdominal pain in childhood is functional, organic causes obviously exist too.

  • Persistent diarrheaDiarrea persistente
  • Recurrent vomitingVomito ricorrente
  • FeverFebbre
  • Blood in the stoolSangue nelle feci
  • Joint pain or swellingDolori o gonfiori articolari
  • Recurrent mouth ulcersAfte ricorrenti nel cavo orale
  • Lesions or fistulas in the perianal areaLesioni o fistole nella regione perianale
  • Associated skin findingsManifestazioni cutanee associate
  • Burning or pain when urinatingBruciore o dolore durante la minzione

Having one or more of these doesn’t automatically mean there’s a serious illness, but it is a reason to talk to the pediatrician, who will decide — based on the history and the examination — whether lab tests or further investigation are needed.

With recurrent abdominal pain, we shouldn’t fall into either of two opposite traps: assuming “it must be psychological,” or automatically putting every child with a tummy ache through a long series of tests. What comes first is the child’s history, growth, diet, physical exam, and a careful check for warning signs — these are what help us decide whether we’re likely looking at ordinary functional pain, or whether there’s reason to dig deeper.

Abdominal pain is one of those symptoms where a good history can really make the difference. When a parent tells me “My child has a tummy ache,” I try not to stop at the symptom itself: I want to know when it started, where it’s located, whether it’s constant or intermittent, whether it moves, when the child last had a bowel movement, whether it gets better afterward, whether there’s vomiting or fever, and above all how the child is behaving.

Sometimes a single piece of information completely changes the picture. Other times the first assessment isn’t enough, and what’s needed is simply to check back after a short interval. That’s why, with abdominal pain, I consider two tools particularly important: a good history, and reassessing over time.

Most of the time, a child’s tummy ache will have a simple cause. But precisely because there are a few rare situations where acting quickly matters, I’d rather make one extra phone call than one too few.
