Headaches are a fairly common complaint in children too.
In the vast majority of cases, it isn’t a sign of a serious illness. Still, especially when episodes keep recurring, it’s important to try to understand what the pain is like, when it appears, how long it lasts, whether it’s changing over time, and whether it comes with other symptoms.
There are also headaches that appear alongside other conditions, for example during a feverish infection, the flu, or rhinosinusitis.
Tension headache generally causes pain or a feeling of pressure or tightness, often on both sides of the head.
The pain is generally mild or moderate and isn’t usually accompanied by nausea or vomiting.
An important feature is that, in most cases, it doesn’t stop the child from carrying on with their normal daily activities.
It can appear more easily as the day goes on, for example after many hours of school or activities, and can be brought on by tiredness, too little sleep, or stressful situations.
The time of day it appears can be a useful piece of information, but on its own, of course, it doesn’t tell us the cause of the headache.
Migraine has different features.L’emicrania presenta caratteristiche differenti.
The pain can start out relatively mild and then grow in intensity. It can be throbbing and affect just one side of the head, but in children it’s fairly common for it to be on both sides.
The child may have:Il bambino può presentare:
- nauseanausea
- vomitingvomito
- sensitivity to lightfastidio per la luce
- sensitivity to noisefastidio per i rumori
- a need to stop what they’re doingnecessità di interrompere le proprie attività
During a significant attack, the child often prefers to lie down in a quiet, dimly lit room, and can feel better after sleeping.
Migraine attacks in children can be shorter than in adults, sometimes lasting just a few hours.
Another important element is family history: very often, other people in the child’s family have, or have had, migraine.
What is migraine with aura?Che cos’è l’emicrania con aura?
In some children, the migraine attack can be preceded by temporary neurological symptoms, most often visual ones.
The child might describe, for example, lights, lines, shimmering images, or other temporary changes in vision.
This is what’s called an aura.È quella che viene chiamata aura.
It shouldn’t be assumed, though, that an aura is necessary to diagnose migraine: most pediatric migraines occur without one.
Headache during an infectionIl mal di testa durante un’infezione
A headache can also appear during a common infectious illness.
A child with fever, the flu, a sore throat, or another viral infection can complain of a headache without this necessarily being a worrying sign.
Rhinosinusitis, too, can cause head pain, sometimes located in the forehead or face.
We shouldn’t automatically conclude, though, that pain in the forehead means sinusitis: it’s the clinical history and the other symptoms that let the pediatrician find their bearings.
When should a headache be a concern?Quando il mal di testa deve preoccupare?
This is probably the most important question for a parent.
Most headaches in children are benign, but there are certain features that call for closer attention.
- appears suddenly and is especially severecompare in maniera improvvisa e particolarmente violenta
- appears after a significant head injurycompare dopo un trauma cranico significativo
- repeatedly wakes the child up at nightsveglia ripetutamente il bambino durante la notte
- is consistently already present on waking, especially with vomitingè già presente sistematicamente al risveglio, soprattutto se accompagnato da vomito
- comes with recurrent vomiting that isn’t easily explainedè associato a vomito ricorrente non facilmente spiegabile
- comes with changes in visionè accompagnato da alterazioni della vista
- comes with difficulty walking or loss of balancesi associa a difficoltà a camminare o perdita dell’equilibrio
- comes with confusion, unusual drowsiness, or significant behavior changesè accompagnato da confusione, sonnolenza insolita o importanti modificazioni del comportamento
- comes with weakness or altered sensation in part of the bodysi associa a debolezza o alterazioni della sensibilità di una parte del corpo
- is associated with seizuresè associato a convulsioni
- appears together with fever, significant neck stiffness or pain, and a decline in overall conditioncompare insieme a febbre, importante rigidità o dolore del collo e condizioni generali compromesse
- is steadily increasing in frequency or intensityaumenta progressivamente di frequenza o intensità
A pain that always appears in exactly the same spot on the head also deserves to be mentioned to the pediatrician, without this necessarily meaning there’s a serious illness.
Fever and headache: should I worry?Febbre e mal di testa: bisogna preoccuparsi?
Not necessarily.Non necessariamente.
It’s very common for a child with a viral infection and fever to also have a headache.È molto comune che un bambino con un’infezione virale e febbre presenti anche mal di testa.
What matters above all is how the child is doing.
A child with 101°F of fever and a headache, who is alert, responds normally, drinks, and behaves in a way that fits with an ordinary feverish illness, is very different from a child with intense headache, neck stiffness, vomiting, unusual drowsiness, or a significant decline in overall condition.
So it isn’t just the number on the thermometer that determines how serious the situation is.Non è quindi il semplice numero indicato dal termometro a stabilire la gravità della situazione.
What about young children?E nel bambino piccolo?
The younger the child, the harder it can be to understand exactly what they’re feeling.
An older child can describe where the pain is, whether it throbs, whether light bothers them, and whether they feel nauseous.
A young child has much more trouble doing that.
For this reason, a recurring or unexplained headache in a young child deserves a pediatric evaluation, without necessarily setting a strict age cutoff.
What can I do at home?Che cosa posso fare a casa?
When a child has an episode of a headache pattern that’s already known, and shows no warning signs, some simple measures can be very helpful.
You can:È possibile:
- let them rest in a quiet, dimly lit roomfarlo riposare in un ambiente tranquillo e poco illuminato
- encourage them to sleep, especially if migraine is suspectedincoraggiarlo a dormire, soprattutto se si sospetta un’emicrania
- offer them water in case they haven’t been drinking enoughoffrirgli acqua se potrebbe avere bevuto poco
- offer them something to eat if they’ve skipped a mealoffrirgli qualcosa da mangiare se ha saltato un pasto
- apply a cool cloth to the forehead, if it brings reliefapplicare, se gli dà sollievo, un panno fresco sulla fronte
When needed, acetaminophen or ibuprofen can be used, at the appropriate dose for the child and when there are no contraindications to their use.
Aspirin, on the other hand, should never be given on your own to a child or teenager, particularly in the context of viral illnesses.L’aspirina non deve invece essere utilizzata autonomamente nel bambino o nell’adolescente, in particolare nel contesto di malattie virali.
Watch out for using painkillers too oftenAttenzione all’uso troppo frequente degli antidolorifici
⚠ Medication overuse can be part of the problemL’uso eccessivo di farmaci può far parte del problema
If a child has frequent headaches, it isn’t a good idea to simply keep giving a painkiller over and over. Using headache medication too often can itself contribute to the headache persisting or becoming chronic. If a child often needs acetaminophen, ibuprofen, or other medication to control the pain, it’s worth talking to the pediatrician about it.Se un bambino presenta mal di testa frequentemente, non è opportuno limitarsi a somministrare ripetutamente un analgesico.
What can trigger a headache?
Especially in children prone to migraine, certain factors can make an attack more likely.
Among the most common:Tra quelli più comuni troviamo:
- skipping mealssaltare i pasti
- not drinking enoughbere troppo poco
- sleeping too littledormire poco
- stressstress
- tirednessstanchezza
- certain changes to the usual routinealcuni cambiamenti della normale routine
Certain foods, too, are sometimes reported as possible triggers by individual patients.
I don’t think it’s a good idea, though, to preemptively cut certain foods from a child’s diet just because they get headaches.
If parents instead repeatedly and convincingly notice that the same food comes before attacks, this information can be reported to the pediatrician and looked into.
Good habits can helpLe buone abitudini possono aiutare
For a child who frequently has headaches, some simple habits can matter a great deal:Nel bambino che soffre frequentemente di mal di testa, alcune semplici abitudini possono essere molto importanti:
eating regularly, drinking an adequate amount of water, sleeping enough with fairly consistent schedules, and getting regular physical activity.mangiare regolarmente, bere una quantità adeguata di acqua, dormire sufficientemente mantenendo orari abbastanza costanti e svolgere regolarmente attività fisica.
It’s also important to consider any stressful situations.È importante anche considerare eventuali situazioni di stress.
In a school-age child, for example, it can help to understand whether the episodes have any relationship to school, commitments, anxiety, or particular times of day.
This doesn’t mean concluding at all that the pain is “psychological.”Questo non significa affatto concludere che il dolore sia “psicologico”.
It means trying to understand all the factors that can contribute to the headache appearing.
When episodes keep recurring, one very simple tool can be extremely useful: the headache diary.
For a few weeks, a parent can note down:Per alcune settimane il genitore può annotare:
- the day and time of the episodegiorno e ora dell’episodio
- how long it lasteddurata
- which part of the head was affectedzona della testa interessata
- how intense the pain wasintensità del dolore
- what the child was doing when it startedche cosa stava facendo il bambino quando è iniziato
- any symptoms that came with iteventuali sintomi associati
- how much they had sleptquanto aveva dormito
- whether they had eaten and had enough to drinkse aveva mangiato e bevuto regolarmente
- any medication giveneventuali farmaci somministrati
- how quickly the pain improvedquanto rapidamente il dolore è migliorato
This information can be far more useful to the pediatrician than simply saying:Queste informazioni possono essere molto più utili al pediatra della semplice affermazione:
“My child has had headaches a lot lately.”“Ultimamente mio figlio ha spesso mal di testa.”
The diary can also help identify triggers that weren’t obvious at first.
What will the pediatrician do?Che cosa farà il pediatra?
When I examine a child for a headache, the first thing I look at is their overall condition.
An alert, responsive child who interacts and moves normally is naturally very different from a child who’s listless, confused, or behaving unusually.
Right after that, one of the most important parts of the visit is a very thorough history.
It’s important to reconstruct when the episodes started, how long they last, how often they happen, whether they’re growing more intense, what time of day they appear, and above all whether they wake the child at night or are already present on waking.È importante ricostruire quando sono iniziati gli episodi, quanto durano, con quale frequenza si presentano, se stanno aumentando di intensità, in quale momento della giornata compaiono e soprattutto se svegliano il bambino durante la notte oppure sono già presenti al risveglio.
I’m also interested in how much the child sleeps, how they’re eating and drinking, whether there are any possible stressful situations, and whether anyone in the family has migraine.
After that, a general and neurological exam is carried out, also checking gait, the eyes and neurological function, and monitoring parameters such as blood pressure, heart rate, and breathing rate.
When the situation calls for it, the pediatrician also looks for any signs that could suggest neurological or meningeal involvement.
Is a CT scan or MRI always needed?Serve sempre una TAC o una risonanza magnetica?
No.No.
This is an important point.Questo è un punto importante.
A child with a headache shouldn’t automatically be sent for a CT scan or an MRI.Un bambino con mal di testa non deve automaticamente essere sottoposto a TAC o risonanza magnetica.
In most cases, a thorough clinical history and a normal exam are already enough for the pediatrician to find their bearings and decide whether further tests are needed.
Imaging is requested when the child’s history, how the headache is evolving, or what comes up during the exam raise elements that deserve closer investigation.
When a child is brought to the pediatrician because of a headache, the most important question isn’t simply:Quando un bambino viene portato dal pediatra perché ha mal di testa, la domanda più importante non è semplicemente:
“How much does it hurt?”“Quanto gli fa male?”
We need above all to understand the story behind that headache.
When did it start?Quando è cominciato?
When does it appear?Quando compare?
Does it wake the child up at night?Lo sveglia durante la notte?
Is it already there when they open their eyes in the morning, or does it appear after they’ve gotten up and started their day?È già presente quando apre gli occhi al mattino oppure compare dopo che si è alzato e ha iniziato le sue attività?
Is it becoming more frequent?Sta diventando più frequente?
Is it always the same, or is it changing?È sempre uguale oppure sta cambiando?
Between episodes, is the child completely well?
It’s this information, together with the exam, that in most cases makes it possible to tell an ordinary headache apart from a situation that deserves further investigation.
For parents, the message can therefore be simple:Per il genitore il messaggio può quindi essere semplice:
headaches in children are common and, in most cases, aren’t caused by a serious illness. What matters is watching how they change over time and recognizing the changes or associated symptoms that deserve a medical evaluation.il mal di testa nei bambini è frequente e nella maggior parte dei casi non è provocato da una malattia grave.
