Ear pain is an extremely common situation in children.
In most cases there are two main causes: acute middle ear infection and outer ear infection.
These are two different situations, affecting different parts of the ear, and they also need different treatments.
More rarely, pain can be caused by an injury, a foreign object, or other conditions.
For this reason, especially when the pain is significant or persistent, the child should be evaluated by the pediatrician.
Middle ear infectionL’otite media
Middle ear infection affects the part of the ear located behind the eardrum.L’otite media interessa la parte dell’orecchio situata dietro la membrana timpanica.
It can affect one ear or both.Può interessare un solo orecchio oppure entrambi.
It very often appears during or after an upper respiratory infection.
A cold, rhinitis, or rhinosinusitis with heavy secretions can interfere with normal ventilation of the middle ear and help trigger the infection.
The most typical symptom is naturally pain, which can be more or less intense and can come with fever.
The pain often becomes especially noticeable when the child lies down, which is exactly why many ear infections seem to appear suddenly at night.
In infants, an ear infection can be harder to recognizeNel lattante l’otite può essere più difficile da riconoscere
An older child can say clearly:Un bambino più grande può dire chiaramente:
“My ear hurts.”“Mi fa male l’orecchio.”
An infant or very young child, of course, can’t do that.
At this age, a middle ear infection can simply show up as greater fussiness and irritability, especially when the baby is put down to sleep.
General symptoms can also appear, such as:Possono comparire anche sintomi generali come:
- feverfebbre
- vomitingvomito
- reduced appetiteriduzione dell’appetito
- irritabilityirritabilità
- reduced usual livelinessriduzione della normale vivacità
- drowsiness or sluggishnesssonnolenza o torpore
These are non-specific symptoms, but in a young child they can accompany a middle ear infection.
Conjunctivitis and ear infection can appear togetherCongiuntivite e otite possono comparire insieme
In a young child, an upper respiratory infection can sometimes come with both conjunctivitis and a middle ear infection at the same time.
That’s why, when a young child with a bad cold has conjunctivitis with purulent discharge, it can be useful during the exam to also check the eardrums, even if the parent hasn’t yet mentioned obvious ear pain.
Outer ear infection is differentL’otite esterna è diversa
Outer ear infection instead affects the ear canal, the part of the ear that lies before the eardrum.L’otite esterna interessa invece il condotto uditivo esterno, cioè la parte dell’orecchio che si trova prima del timpano.
It’s especially common during periods when children swim a lot, at the sea or in a pool.È particolarmente frequente nei periodi nei quali i bambini fanno molti bagni al mare o in piscina.
Moisture that stays in the ear canal can irritate the skin and later lead to an infection.L’umidità che rimane nel condotto uditivo può infatti favorire l’irritazione della cute e successivamente un’infezione.
Unlike middle ear infection, outer ear infection is less often associated with fever.A differenza dell’otite media, l’otite esterna è meno frequentemente associata a febbre.
A fairly distinctive feature is pain triggered by gently moving or pulling on the outer ear, or by pressing at the entrance of the ear canal.
With middle ear infection, this kind of maneuver generally doesn’t cause the same pain.
It’s a useful clue, but of course it doesn’t replace an otoscopy.È un’indicazione utile, ma naturalmente non sostituisce l’otoscopia.
When fluid comes out of the earQuando esce del liquido dall’orecchio
Sometimes a middle ear infection can cause the eardrum to rupture on its own.
In this case, a parent may notice mucous or purulent material coming out of the ear.
Sometimes it’s noticed in the morning as a stain on the pillowcase.
Something that can seem paradoxical can happen: right when the discharge appears, the pain can suddenly ease.
This happens because the rupture relieves the pressure that had built up behind the eardrum.
⚠ Less pain doesn’t mean it’s resolvedMeno dolore non significa risolto
The pain easing doesn’t necessarily mean the problem is over. A child with discharge from the ear still needs to be seen by a doctor.
After an ear infection, a child may hear lessDopo l’otite il bambino può sentire meno
After an episode of middle ear infection, fluid can remain in the middle ear for some time.
A parent may then get the impression the child is hearing less than usual.
In most cases this tends to resolve gradually on its own.
There are children, though, in whom the fluid lingers for longer.
This can happen, for example, when enlarged adenoids interfere with normal ventilation of the middle ear.
This can lead to otitis media with effusion: generally little or no pain, but able to cause a reduction in hearing that can persist for a while.
So when a parent has the impression that a child still isn’t hearing well after an ear infection, it’s worth mentioning to the pediatrician.
What can I do right away if their ear hurts?
This is a very common question, especially when the pain shows up in the middle of the night.È una domanda molto frequente, soprattutto quando il dolore compare nel cuore della notte.
The first thing to do is control the pain.
Medications such as acetaminophen or ibuprofen can be used, at the appropriate dose for the child.
Ibuprofen should be avoided when there’s possible dehydration, for example in a child who is vomiting repeatedly, has significant diarrhea, or is taking in very little fluid.L’ibuprofene deve essere evitato quando esiste una possibile disidratazione, per esempio in un bambino che vomita ripetutamente, ha una diarrea importante o assume pochissimi liquidi.
Pain relief often makes it possible to get through the first hours and wait for the pediatrician’s evaluation, as long as the child’s overall condition is good and there are no other worrying signs.
Let’s avoid putting drops in the ear without knowing what’s in thereEvitiamo di mettere gocce nell’orecchio senza sapere che cosa c’è
Faced with ear pain, it can be tempting to use ear drops already at home.
It’s better to avoid this without a prior evaluation.È meglio evitarlo senza una precedente valutazione.
Before using certain medications in the ear canal, it’s important to know whether the eardrum is intact.
Not every topical preparation is suitable when there’s a perforation of the eardrum.
And before prescribing a local treatment, the doctor needs to determine whether we’re really dealing with an outer ear infection or a middle ear infection.
That’s why the otoscopy matters.Per questo l’otoscopia è importante.
First we look at the ear, then we decide what to put inside it.Prima guardiamo l’orecchio, poi decidiamo che cosa metterci dentro.
If there’s discharge, keep the ear drySe c’è otorrea, manteniamo l’orecchio asciutto
When there’s discharge coming from the ear, it’s best to keep water out of the ear canal until it’s been evaluated and has healed.
During this phase, swimming pools and sea dips should be avoided, and care should also be taken when washing the child’s hair.
The pediatrician will assess the cause of the discharge, the treatment needed, and whether a follow-up check of the eardrum is appropriate.
Do all ear infections need an antibiotic?Tutte le otiti richiedono un antibiotico?
No.No.
The decision depends on the child’s age, their overall condition, fever, how intense and how long the symptoms have been, whether one or both ears are involved, and above all what the doctor sees during the otoscopy.
In some cases of middle ear infection, it can initially be enough to control the pain well and watch how things develop.
In other situations, the pediatrician will consider antibiotic treatment appropriate.
Outer ear infection is different again, and can call for a specific topical treatment.L’otite esterna è ancora differente e può richiedere una terapia locale specifica.
For this reason, it isn’t a good idea to use, on your own, an antibiotic left over from a previous infection.
Ear pain during air travelIl mal d’orecchio durante un viaggio in aereo
A particular problem is ear pain during takeoff and, especially, during landing.
Changes in atmospheric pressure can create a pressure difference between the middle ear and the outside environment.
A simple trick can be to have the child drink during the phases when pressure is changing, when their age allows for it.Un accorgimento semplice può essere quello di far bere il bambino durante le fasi di variazione della pressione, quando l’età lo consente.
Swallowing helps open the Eustachian tube and helps rebalance the pressure in the middle ear.
In an infant, nursing or sucking during these phases of the flight can also help, in keeping with their normal feeding rhythm.
When should I call the pediatrician?Quando devo chiamare il pediatra?
Ear pain deserves the pediatrician’s opinion especially when the pain is significant or persistent, fever appears, the child is very young, or their overall condition isn’t good.
⚠ It’s also worth calling when:È opportuno contattarlo anche quando:
- discharge appears from the earcompare secrezione dall’orecchio
- the pain isn’t adequately controlled by pain relieversil dolore non viene adeguatamente controllato dagli antidolorifici
- the child seems especially unwellil bambino appare particolarmente abbattuto
- there’s repeated vomiting or other significant symptomssono presenti vomito ripetuto o altri sintomi importanti
- a parent notices persistent reduced hearingil genitore nota una persistente riduzione dell’udito
- ear infection episodes tend to happen frequentlygli episodi di otite tendono a ripetersi frequentemente
In infants, the threshold for asking for advice should naturally be lower, since symptoms can be much less specific.
Ear pain is one of those symptoms that often show up at the least convenient moment: frequently at night.
A child’s first need is to not be in pain.
That’s why, when the child’s overall condition is good and there are no other worrying signs, we can first use an appropriate pain reliever and then have the child evaluated.
What I would avoid, instead, is trying to treat the ear directly without knowing what’s happening behind or in front of the eardrum.
A middle ear infection and an outer ear infection can both cause pain, but they’re two different problems.
And discharge that suddenly comes out of the ear can even come with the pain going away, without that meaning the infection has healed.E una secrezione che improvvisamente esce dall’orecchio può perfino accompagnarsi alla scomparsa del dolore, senza significare che l’otite sia guarita.
So the principle stays simple:Per questo il principio rimane semplice:
first we control the pain, then we look at the ear, and only after that do we decide on the most appropriate treatment.prima controlliamo il dolore, poi guardiamo l’orecchio e soltanto dopo decidiamo la terapia più appropriata.
