My Child Isn’t Pooping: Is It Really Constipation?

Constipation is an extremely common problem in pediatrics.

When a parent tells me, “My child isn’t pooping,” the first thing to understand is whether this is really constipation.

How often a child has a bowel movement varies enormously with age, and can also vary considerably from one child to another.

To define constipation, we can’t simply count the days since the last bowel movement. We need to consider, above all, the consistency of the stool, the difficulty and pain involved in passing it, and the child’s behavior.

In the first days and weeks of life, especially if breastfed, a baby may spontaneously have several bowel movements a day.

After the first few weeks, this can change considerably. Some infants continue to go three or four times a day; others may go several days between bowel movements.

Both can be normal, as long as the stool stays soft, the baby goes without particular difficulty, and growth is regular.

A baby who goes rarely but passes soft stool is not necessarily constipated.

Stool changes with the introduction of solid foodsCon il divezzamento le feci cambiano

With the introduction of complementary feeding, the characteristics of the stool inevitably change. It becomes progressively more formed, and some children can start to lean toward constipation.

Several factors come into play: diet, fluid intake, activity, and the child’s habits. A varied, balanced diet, adequate fluids, and normal physical activity all support regular bowel movements.

But once true constipation has already set in, diet and activity alone may not be enough to resolve it.Quando però una vera stitichezza si è già instaurata, alimentazione e movimento possono non essere sufficienti a risolverla.

We don’t just ask: “How often does he go?”Non chiediamo soltanto: “Ogni quanto fa la cacca?”

When I’m assessing a child for possible constipation, I want to know more. Is the stool soft or hard? Is it a large amount, or made up of many small hard pellets? Does the child have to push a lot? Does he cry when going? Does she deliberately try to hold it in? Is there abdominal pain? Has blood appeared after a bowel movement? Are there episodes of accidentally soiling the underwear?

All of this information is often more important than the simple number of bowel movements per week.

Hard stools, pain, and fear of goingFeci dure, dolore e paura di evacuare

One of the most important mechanisms in childhood constipation is a genuine vicious cycle.

The child passes a particularly hard stool and feels pain. The next time the urge comes, he remembers that experience and tries to hold the stool in. But the longer stool stays in the bowel, the harder it becomes.

The next bowel movement will then be even more difficult and painful.

Hard stool → pain → fear of going → holding it in → even harder stool.

A key part of treatment consists precisely of breaking this cycle.

When the child holds it in on purposeQuando il bambino trattiene volontariamente le feci

The behavior can be very distinctive. The parent notices that the child feels the urge but, instead of going, stiffens up, crosses the legs, takes on unusual postures, hides, or tries in every way to hold it in.Il comportamento può essere molto caratteristico.

Sometimes it all starts after a previous painful bowel movement. In other children, the problem can appear during the switch from diapers to the potty, especially when toilet training is introduced too early or becomes a source of excessive pressure.

In both cases, it’s important to avoid letting bowel movements turn into a daily conflict between child and parents.

Toilet training is a skill that matures gradually, and not every child is ready at the same age.

Constantly forcing a child to sit on the potty, scolding them for not going, or showing excessive worry can increase their resistance.

Instead, it helps to build a calm routine. A good time to offer the potty or toilet is after main meals, taking advantage of the natural gastrocolic reflex that promotes a bowel movement after eating.È invece utile creare una routine tranquilla.

When using the toilet, it also helps for the child to have their feet resting firmly on a support, rather than dangling.

Offer it, wait a few minutes, but don’t turn the moment into a test to pass.

Hard stools can also cause bleedingLe feci dure possono provocare anche sangue

Passing particularly hard stool can cause small tears in the anal lining, known as anal fissures.L’emissione di feci particolarmente dure può provocare piccole fissurazioni della mucosa anale, le cosiddette ragadi.

The parent may then notice a small amount of bright red blood on the surface of the stool or on the toilet paper.

A fissure can make bowel movements even more painful, further feeding the child’s fear.

That said, blood should always be interpreted in context, and especially when it doesn’t clearly seem linked to hard stools, it should be reported to the pediatrician.

A build-up of stool in the bowel can cause abdominal pain, bloating, and sometimes reduced appetite. In older children, the pain can also be significant.

For this reason, when I examine a child for abdominal pain, one of the questions I routinely ask is always about the bowels: “When was his last bowel movement? What was it like?”Per questo, quando visito un bambino per dolore addominale, una delle domande che faccio abitualmente riguarda sempre l’alvo: “Quando è andato di corpo l’ultima volta? Come erano le feci?”

In some cases, significant constipation can cause pain that needs to be distinguished from other causes of abdominal pain.

“Diarrhea” that’s actually constipationUna diarrea che in realtà è stitichezza

It may sound contradictory, but a very constipated child can have accidental leaks of loose or liquid stool.

When there is a mass of hard stool in the rectum, softer material can pass around the blockage and leak out involuntarily. The parent sees soiled underwear and may think the child has diarrhea.

In reality it can be exactly the opposite: those leaks can be the result of significant constipation.

Recognizing this is important, because misreading it as diarrhea could lead to doing the opposite of what’s actually needed.

Diet, fluids, and movementAlimentazione, liquidi e movimento

Normal daily physical activity supports bowel motility. It also helps for the child to have a varied, balanced diet, with an adequate amount of fiber-containing foods and enough fluids for their age.

That said, I’d avoid turning the management of constipation into a long list of “laxative” and “binding” foods. Once the problem has become established, changing the diet alone isn’t always enough. It may be necessary to intervene directly on stool consistency.

When stools need to be softenedQuando è necessario ammorbidire le feci

When a child is passing hard, painful stool, or has already developed a pattern of holding it in, one of the fundamental goals of treatment is to achieve consistently soft, easy-to-pass stool.

It isn’t enough to get the child to go just once. We need to let them go without pain for long enough to break the association:Non basta far evacuare il bambino una volta.

bowel movement = pain.evacuazione = dolore.

The first-choice medication is generally macrogol (PEG), an osmotic laxative that keeps stool soft. Treatment may need to continue for several months, according to the pediatrician’s guidance.

The goal is to let the child gradually forget the fear of going, stop holding stool in, and regain a normal bowel rhythm.L’obiettivo è permettere al bambino di dimenticare progressivamente la paura dell’evacuazione, smettere di trattenere le feci e recuperare un ritmo intestinale normale.

Lactulose: a possible second choiceIl lattulosio: una possibile seconda scelta

Lactulose is another recognized treatment for pediatric constipation and can be a second-choice option.

In my practice I use it fairly often, especially in the first years of life, when it can be hard to get a child to take the volume of liquid needed for macrogol.

Since it’s a syrup, it’s generally accepted more easily, and can also be added to milk or other drinks.

It can cause some side effects, mainly bloating and gas, but in some young children the ease of giving it is a real practical advantage.

The choice of medication, dose, and length of treatment should still be decided together with the pediatrician.

Most constipation is functionalLa maggior parte delle stitichezze è funzionale

In the vast majority of children, constipation isn’t caused by an organic disease. This is called functional constipation.

There are, however, some much rarer organic causes that need to be considered, especially when the problem appears very early. These include, for example, Hirschsprung’s disease, certain anorectal abnormalities, and certain neurological conditions affecting the lower spine.

These are completely different situations from common functional constipation, and they require specific medical assessment.

What are the warning signs?Quali sono i segnali d’allarme?

In newborns, it’s particularly important if meconium isn’t passed within the first 48 hours of life.

The following situations also deserve medical evaluation:Meritano inoltre una valutazione medica situazioni come:

⚠ See a doctor if you notice:Consultate un medico se notate:

  • a very distended abdomenaddome molto disteso
  • vomiting, especially if bile-coloredvomito, soprattutto se biliare
  • fever together with a decline in the child’s general conditionfebbre associata a compromissione delle condizioni generali
  • rectal bleeding not easily explained by a simple fissuresanguinamento rettale non spiegabile semplicemente da una ragade
  • significant constipation present from the very first weeks of lifestitichezza importante presente fin dalle primissime epoche della vita
  • stool that is persistently very thin or ribbon-likefeci persistentemente molto sottili o nastriformi
  • poor growth or weight lossscarso accrescimento o perdita di peso
  • urinary incontinence or recurrent urinary infections together with significant constipationincontinenza urinaria o infezioni urinarie ricorrenti associate a una stitichezza importante
  • changes in strength, tone, or movement of the legsalterazioni della forza, del tono o dei movimenti degli arti inferiori

During the visit, the pediatrician may also look for any abnormalities of the anus and lower spine, or other findings suggesting an anatomical, neurological, endocrine, or intestinal cause.

These situations are rare, but it’s important to be able to tell these children apart from the much more common functional constipation.

⚠ Watch out for green vomitAttenzione al vomito verde

In newborns and young children, clearly green vomit can indicate the presence of bile and should be treated as a warning sign. This is not something to manage at home as ordinary constipation: it requires urgent medical evaluation.

When should you contact the pediatrician?Quando è opportuno contattare il pediatra?

It’s a good idea to ask the pediatrician’s advice when constipation is persistent, causes significant pain, the child systematically holds stool in, involuntary fecal leaks keep happening, or the situation isn’t improving despite the usual measures taken at home.È opportuno chiedere il parere del pediatra quando la stitichezza è persistente, provoca dolore importante, il bambino trattiene sistematicamente le feci, compaiono ripetutamente perdite fecali involontarie oppure la situazione non migliora nonostante le normali misure adottate a casa.

Evaluation becomes even more important if there’s blood that isn’t clearly explained by a fissure, vomiting, significant abdominal distension, poor growth, or other associated symptoms.

In newborns and very young infants, the threshold for seeking evaluation should be even lower.

When a parent tells me, “My child isn’t pooping,” I’m not just interested in how many days it’s been.

I want to know what the stool looks like, whether going hurts, and above all what the child does when they feel the urge coming.

Because very often the real problem in childhood constipation isn’t simply the presence of hard stool. It’s the cycle that gradually builds between pain, fear, and holding it in.Perché molto spesso il vero problema della stitichezza infantile non è semplicemente la presenza di feci dure.

That’s why our goal shouldn’t simply be: “Get the child to poop.”Per questo il nostro obiettivo non deve essere semplicemente: “Far fare la cacca al bambino.”

We need to achieve something more important: helping the child go back to passing soft stool, without pain, without fear, and without conflict with their parents.

Sometimes this takes time. And that’s precisely why effective treatment shouldn’t necessarily be stopped as soon as the child starts going again.

We need to give it the time it needs for bowel movements to go back to being a normal function of the body, not something to fear or hold back.
