Cough and cold are among the most common childhood complaints. In most cases they’re linked to ordinary viral infections of the upper airways and clear up on their own.
When a parent tells me “Doctor, my child has a cough,” the first thing I want to know isn’t which syrup they’re using. I want to know how the child is doing, how long the cough has lasted, and what that cough actually sounds like.
First things first: how is the child, and what kind of cough is it?Prima di tutto: come sta il bambino, e che tipo di tosse è?
As always in pediatrics, we need to look at the child before we look at the symptom. A child who’s coughing but still playing, eating, drinking and full of energy is very different from a child who looks pale, listless, is struggling to breathe, or has significantly changed their eating habits. That’s why one of the questions I ask parents is: “Is he eating as usual?” A significant drop in appetite, especially alongside a general decline in how the child seems, is information worth paying attention to.Come sempre in pediatria, prima del sintomo dobbiamo guardare il bambino.
Not all coughs are the same. It’s important to work out whether it’s a dry or a wet (productive) cough, whether it shows up mainly during the day or at night, and under what circumstances it appears. A cough that gets worse when the child lies down can be caused, for example, by nasal secretions draining down the back of the throat — one of the most common causes of cough linked to a cold, especially in young children. In other children, a cough that’s mainly nocturnal can also suggest a bronchospastic component: that’s why it can be useful to know the child’s history — has he had atopic dermatitis or other allergic manifestations? Is there asthma or allergies in the family? Even in front of a simple cough, a good history can make a real difference.
How long does a cold normally last?Quanto dura normalmente un raffreddore?
The common cold is an acute rhinitis, usually viral in origin. The word acute matters: it means the illness has a beginning and an end. In most cases it tends to resolve over about 10-14 days. Early on, secretions can be clear and runny, later becoming thicker and even yellowish — the color of the secretions on its own doesn’t necessarily mean a bacterial infection has set in. What matters most is how things evolve over time.
If, after about two weeks, the child still has significant nasal secretions and cough — especially a wet cough that worsens when lying down — the picture deserves a fresh look. In some cases this is no longer the simple evolution of a viral rhinitis, and a rhinosinusitis may have developed. This can happen more easily in children with enlarged adenoids, since the adenoids can hinder normal drainage of secretions and encourage them to pool. In these situations, it’s up to the pediatrician, after examining the child, to decide whether specific treatment is needed: the duration of the cough or the color of the mucus, on their own, are not a reason to start antibiotic treatment on your own.
The pattern of the fever also deserves attention. A fever present in the first days of a respiratory infection is very common with viral illnesses. It’s a different situation when a child starts with a fever, is fever-free for a few days, and then spikes a fever again: this pattern deserves closer attention, because it can point to a complication or a superinfection, and it’s worth having the child reassessed.
When should we worry about breathing?Quando dobbiamo preoccuparci del respiro?
More than the cough itself, what I want to see is how the child is breathing. Pay particular attention if he:Più della tosse in sé, mi interessa osservare come respira il bambino. Bisogna prestare particolare attenzione se:
- Is breathing much faster than usualRespira molto più velocemente del solito
- Seems to be working hard to breatheSembra fare fatica a respirare
- Shows visible chest retractions while breathingPresenta evidenti rientramenti del torace durante la respirazione
- Looks pale or has an unusual color around the lipsAppare pallido o presenta una colorazione anomala delle labbra
- Is very listlessÈ molto abbattuto
- Can’t talk, drink, or feed normally because of breathing difficultyNon riesce normalmente a parlare, bere o alimentarsi a causa della difficoltà respiratoria
In these situations the child needs to be seen quickly.
The barking coughLa tosse abbaiante
There’s a cough that most parents, once they’ve heard it, learn to recognize easily: a dry, barking cough, often with hoarseness or a lower voice, and sometimes a noise on breathing in called stridor. This is the classic picture of subglottic laryngitis (croup), a common viral infection of the upper airways in young children that characteristically tends to appear or worsen at night. Most episodes follow a mild, favorable course. Stridor present even when the child is calm, breathing difficulty, pallor, cyanosis, unusual drowsiness, or rapid worsening call instead for an urgent evaluation.
Not all coughs are the same.Non tutte le tossi sono uguali.
A cough that starts suddenly deserves one more questionUna tosse comparsa improvvisamente merita una domanda in più
There’s a situation we shouldn’t forget, especially in the first years of life: a child who was fine suddenly starts coughing. In this case I always ask: “What was he doing when the cough started?” If the cough appeared suddenly during a meal, or while the child had a small piece of food or an object in his mouth, and especially if it was preceded by a choking episode, the possibility of an inhaled foreign body needs to be considered. The child may even seem to feel better after that first episode and simply keep coughing — which is exactly why the story of what happened at the precise moment the cough began can be decisive.C’è una situazione che non dobbiamo dimenticare, soprattutto nei primi anni di vita: un bambino stava bene e improvvisamente comincia a tossire.
Is there a cure for the common cold?Esiste una cura per il raffreddore?
This is one of the questions parents ask me most often: “Doctor, what can I give him to make the cold go away?” The answer needs to be very clear: at present there is no truly effective treatment that cures acute viral rhinitis or makes it disappear faster. A cold has its own natural course: it starts, evolves, and in most cases resolves on its own over about 10-14 days. That’s why we don’t necessarily need to give a medication every time a child has a stuffy or runny nose: many products marketed to “cure” a cold don’t really change its course and, especially in young children, can be useless or cause unwanted effects.
What we can do is try to make the child more comfortable during the days it takes to get better. In young children it can help to keep the nose clear with saline solution, especially before meals and before sleep, when nasal congestion tends to cause the most discomfort. We don’t necessarily need to treat every symptom of an illness with a medication: very often the best thing we can do for a common cold is help the child feel a bit better while giving the body the time it needs to heal.
The same caution applies to cough syrups: it’s best to avoid self-medicating, especially in younger children. Cough is a defense mechanism — among other things, it helps clear secretions from the airways. Before simply trying to suppress it, we need to understand why the child is coughing. It’s also worth noting that some cough-and-cold products combine several active ingredients: before giving them, it’s important to check they’re appropriate for the child’s age and to read the composition carefully.

When a parent tells me “My child has had a cough for two weeks,” those two weeks matter, but they’re not enough on their own. I want to know how the cough started, whether it’s dry or wet, when it shows up, whether the child has had a fever, whether the fever went away and then came back, whether he has a cold, and above all how the child is doing. Sometimes it’s exactly a seemingly minor question that changes the whole picture: a cough that started during a cold and a cough that started suddenly while a child was eating can look, a few days later, like simply “a cough” — but their stories are completely different. That’s why, even with a symptom as common as this one, a good history and careful observation of the child usually remain the most important tools we have.
