Vulvovaginitis in Girls: What to Do

Vulvovaginitis is a very common problem in young girls, especially before puberty.La vulvovaginite è un problema molto frequente nelle bambine, soprattutto prima della pubertà.

It shows up as inflammation of the vulva, meaning the external genitals, sometimes with the vagina involved too. Why is vulvovaginitis so common in young girls?Perché la vulvovaginite è così frequente nelle bambine?

The tissues are thinner and less protected by the effect of estrogen. On top of that, how close the anus and vulva are to each other makes it easier for fecal material and bacteria to reach the genital area.I tessuti sono più sottili e meno protetti dall’azione degli estrogeni. On top of this come some very common childhood habits: not-yet-perfect cleaning after using the bathroom, bubble baths or irritating cleansers, tight-fitting clothes, moisture, and friction.A questo si aggiungono comportamenti molto comuni nell’infanzia: una pulizia non ancora perfetta dopo essere andate in bagno, l’uso di bagnoschiuma o detergenti irritanti, indumenti aderenti, umidità e sfregamento.

What are the most common causes?Quali sono le cause più frequenti?

In these cases, there isn’t necessarily a specific germ responsible for the inflammation.In questi casi non è necessariamente presente un germe responsabile dell’infiammazione.

In some cases, a bacterial infection can be present instead.In alcuni casi può invece essere presente un’infezione batterica.

Young girls can easily carry bacteria to the vulvar area from the anus, or, through their hands, from the mouth or nose.Le bambine possono facilmente trasportare nella regione vulvare batteri provenienti dall’ano oppure, attraverso le mani, dalla bocca o dal naso.

That’s why, when I examine a girl with vulvovaginitis, I’m also interested in knowing whether she’s had fever, a sore throat, or other signs of infection in the previous days.Per questo, quando visito una bambina con vulvovaginite, mi interessa anche sapere se nei giorni precedenti abbia avuto febbre, mal di gola o altre manifestazioni infettive.

What about Candida?E la Candida?

Actually, vulvovaginal candidiasis is relatively rare in girls before puberty.In realtà la candidosi vulvovaginale è relativamente rara nella bambina prima della pubertà.

It can be more likely in the presence of certain predisposing conditions, for example after certain antibiotic treatments or in children with particular medical conditions.Può essere più probabile in presenza di particolari condizioni predisponenti, per esempio dopo alcune terapie antibiotiche o in bambini con determinate condizioni mediche.

At night, the females lay eggs in the area around the anus and can cause very intense itching.Durante la notte le femmine depongono le uova nella regione intorno all’ano e possono provocare un prurito molto intenso.

The irritation can also spread to the vulvar area.L’irritazione può interessare anche la regione vulvare.

One particularly useful thing to ask, then, is:Un elemento particolarmente utile è quindi chiedersi:

If the girl scratches mostly during sleep, or also has itching around the anus at the same time, it’s worth mentioning to the pediatrician.Se la bambina si gratta prevalentemente durante il sonno o presenta contemporaneamente prurito anale, è opportuno riferirlo al pediatra.

Could there be a foreign object?Può esserci un corpo estraneo?

What are the symptoms?Quali sono i sintomi?

It isn’t necessary for all of these symptoms to be present at the same time.Non è necessario che siano presenti tutti questi sintomi contemporaneamente.

Burning when urinating: is it always a bladder infection?Bruciore quando fa pipì: è sempre cistite?

No.No.

When the skin of the vulva is very irritated, urine simply coming into contact with the inflamed area can cause burning while urinating.Quando la cute della vulva è molto irritata, il semplice contatto dell’urina con la zona infiammata può provocare bruciore durante la minzione.

This doesn’t necessarily mean the girl has a urinary tract infection.Questo non significa necessariamente che la bambina abbia un’infezione delle vie urinarie.

It isn’t a good idea, though, to put creams, gels, or other products inside the vagina on your own.Non è invece opportuno introdurre autonomamente creme, gel o altri prodotti all’interno della vagina.

It may seem like a minor detail, but it isn’t.Può sembrare un particolare di poca importanza, ma non lo è.

When the picture looks consistent with irritant or nonspecific vulvovaginitis, the first step is often simply correcting hygiene, removing possible irritants, and protecting the skin.Quando il quadro appare compatibile con una vulvovaginite irritativa o aspecifica, spesso il primo provvedimento consiste semplicemente nel correggere l’igiene, eliminare i possibili irritanti e proteggere la cute.
